OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT. BY COMPLETING THIS DOCUMENT YOU WILL
WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ THIS
CONTRACT CAREFULLY
The undersigned is aware that there are certain
inherent risks involved in participating in Toledo Sport and Social Club, LLC including but not limited to
the risk of theft or of damage to my property, and the risk of personal injury from participating in athletic
activities. In consideration of my being granted permission to participate in these activities to use the
facilities of the City of Toledo, Sport & Health Clubs, and other private schools, and/or other activities
provided by the Toledo Sport and Social Club, LLC, I, on behalf of myself, my executors, administrators, heirs,
next of kin, and successors, hereby covenant to hold harmless and indemnify Toledo Sport and Social Club, LLC
from any and all claims, losses, damages, injuries, fines, penalties, and costs (including court costs and
attorney’s fees), charges liabilities, or exposures, however caused, resulting from or arising out of or in
any way connected to my or my family’s participation in the above described activity.
Participation in a Toledo Sport and Social Club activity infers you have read and agree to these terms.
SPORTSMANSHIP AND
FAIR PLAY AGREEMENT:
1. I have read, understood
and will abide by the rules of my sport as outlined by TSSC.
2. I understand
that if it is reported by the Leauge Official or an opponent that I have behaved in an
unsporting manner or have not played by the rules, that I could be suspended or
ejected from the league, with no refund.
3. I understand
that my team captain is the only person who can approach the League Official or the opposing
captain regarding concerns surrounding the game. It is my responsibility to let
my captain know my concerns so he/she can deal with them.
